A Bremen man was killed in a head-on collision Monday night in Cullman County.

Alabama State Troopers identified the fatality victim as Cody L. Self. He was 33.

The crash happened at 7:34 p.m. on Cullman County 38 near Cullman County 46, about three miles south of Dodge City.

Senior Trooper Brandon Bailey said Self was driving a Mazda 31 that collided head on with a GMC Yukon driving by Megan D. Swidecki, 38, of Hanceville.

Self was not using a seat belt and was pronounced dead on the scene.

Four passengers of the GMC were injured. Those hurt were 61-year-old Kelley M. Waunch and three children ages 10, 8 and 7. They were taken to area hospitals for treatment.

The crash remains under investigation by troopers.
